The ADT Nurse Specialist (admission, discharge, and transfer) is a registered nurse who demonstrates clinical expertise through advanced education and/or experience, knowledge and skills in area of practice and clinical nursing specialization. The ADT Nurse Specialist is primarily responsible for consulting and advising on assessments and documentation for arrivals and departures on our identified  unit. The ADT Nurse Specialist will assist with admission assessments, discharge instructions, and facilitate patient transfers off of the floor to ensure timely transfers and patient satisfaction. The ADT Nurse Specialist ensures the delivery of patient care supports the mission, philosophy, and goals of the Department of Nursing and Hackensack University Medical Center.

Responsibilities

A day in the life of an ADT nurse specialist at Hackensack Meridian Health may include:

  • Provides patient care, applying advance clinical skills and critical thinking to ensure optimal outcomes of clinical outcomes
  • Provides support and acts as a clinical resource for staff: monitor clinical practice to ensure compliance with established standards
  • Accepts responsibility and accountability for assessing the appropriate patient care through assessment, implementation, and ongoing evaluation of the delivery of patient care
  • Demonstrates professional involvement by participating in workshops, journal club, seminars, and grand rounds fostering review and discussion of research current articles as a presenter or member
  • Participates in the development and implementation of continuous quality improvement programs, QA/PI activities ie: including but not limited to chemotherapy verification process
  • Applies evidence based practice to make clinical decisions and establish standards of clinical specialty
  • Participates directly or supports the professional nursing organization and shared clinical decision making process
  • The nurse demonstrates the use of nursing process as a problem solving model; utilizing their knowledge and experience to anticipate and plan to meet patient and family needs
  • Nursing assessment is focused to target areas most productive for a given patient population or situation. Recognizes subtle changes in clinical situation; evaluates and appropriately alters the plan of care. Is accountable for prioritizing and organizing patient care and ensuring follow through with planned care
  • Completes the admission assessment in the EMAR, Allergy Assessment, Home medication assessment, and health history on scheduled, unscheduled, and direct patient admissions on Medical Surgical / Telemetry units and for Med-Surg/ Tele patients holding in ICU, ED, and PACU
  • Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders: completes the discharge summary, provides patient education, and documents discharge teaching
  • Communicates with the RN that is responsible for the patient what aspects of the discharge process has been completed
  • Communicates with nursing personnel to assist in the admission, discharge, and transfer of in-patients
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served on the assigned unit. Consistently applies current literature/research findings and critical thinking skills to make sound clinical decisions
  • Demonstrates the Hospital's Patient Satisfaction effort when interacting with patients, families, and co-workers
  • The nurse develops and maintains a therapeutic nurse/patient relationship throughout the health care continuum. Respect for patient's rights is an integral part of the nurse/patient relationship as reflected in our values and mission. Conducts hourly rounding on patients

 

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Minimum of Bachelor's degree in Nursing from an accredited nursing program and five years of professional nursing experience, administrative/supervisory experience.
  • Knowledge of team making management practices
  • Knowledge of nursing principles, practices, and techniques
  • Knowledge of supervisory practices and principles
  • Knowledge of state and agency laws and regulations governing general professional nursing practices
  • Skill in both verbal and written communication
  • Skill in leadership
  • Skill in applying professional nursing methods and techniques
  • Skill in maintaining effective working relationships.

Education, Knowledge, Skills and Abilities Preferred:

  • Master's Degree
  • Certification in Nursing Administration desirable.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License
  • AHA Basic Health Care Life Support HCP Certification

Licenses and Certifications Preferred:

  • Advanced Cardiac Life Support Certification
