The Trauma Registrar is responsible for abstracting required data elements from the medical record to the data collection form or for computer database input. Maintains database, generates reports and submits data to NJ State trauma registry system.

Responsibilities

A day in the life of a Trauma Registrar at Hackensack Meridian Health includes:

 

  • Analyzes medical records to ensure compliance with medical record policy. Serves as a liaison between trauma and medical records with particular issues of chart accessibility and coding accuracy. Provides additional quality review for medical records billing
  • Codes records: Determine principal and secondary diagnoses and treatment procedures; codes using standardized coding systems. Consults medical and coding reference books as necessary to determine most appropriate code; enters data info trauma registry database as required.
  • Documentation of billing: Responsible for completion and submission of billing documentation upon patient discharge. Facilitates resolution to billing inquiries. Accounts are submitted to third parties and patients with accurate charge, insurance and demographic information.
  • Maintenance of database/ equipment. Operates office equipment properly and makes recommendations for maintenance and replacement of supplies/equipment. Expert knowledge of computerized MR systems and other related systems required to perform daily tasks.
  • Medical record management: Identifies medical records for trauma registry entry throughout the hospital. Performs performance improvement issue identification and referral on medical records of trauma patients post-discharge.
  • Office chart management: Assembles and maintains all patient office charts including all dictations and additional billing information. Charts are filed and contain accurate, current documentation. Charts are easily retrievable and available when requested for resolving issues.
  • Produces reports: Produces reports from database in requested format to assist hospital and trauma center management. Ensures that all requests for information are addressed appropriately and completed within an acceptable time frame.
  • Plans, organizes, directs and coordinates work of the registrar(s) to ensure maximum productivity and accuracy of ICD, CPT and AAAM codes. Maintains a well-organized and efficient registry. Performs periodic quality audits on registry data post-discharge.
  • Interacts with patients, families, medical professionals and in-house medical staff. Abstracts any requested trauma date to aid other departments in utilization analysis and management of resources. Communicates in a clear, concise manner.
  • Demonstrates the ability to update and maintain databases as needed. Communicates effectively with vendor representatives. Keeps staff informed of changes which would affect the operation of the trauma registry.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
  • Experience in a healthcare setting.
  • Knowledge of medical terminology.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
